The care manager will collaborate with practice providers to identify patients in need of care coordination and develop individual care plans with patient-specific goals and interventions. Duties include performing comprehensive patient assessments, coordinating referrals, patient education, clinical documentation, reviewing clinical data and metrics, and serving as a patient liaison and advocate with local hospitals/health systems, nursing centers, home health, and community providers. The position requires an active RN licensure with the Maryland Board of Nursing. Candidates must have at least three years of previous clinical experience and possess strong clinical assessment and chronic disease management skills. Knowledge of population health and value-based programs is preferred.

This position will manage daily operations and perform analysis within the regulatory requirements.
